Dr Calloway has looked after hearts in central Texas since 2002. He trained in interventional cardiology at Brookline Community Hospital and now runs the cardiac catheterisation laboratory at Cedar Park Medical Center, where he places around three hundred stents a year. Most of his week, though, is ordinary clinic work: chest pain that turns out to be reflux, blood pressure that will not settle, people who feel breathless on the stairs and are frightened about what it means. He is known for drawing the problem on paper before he explains it, for saying plainly when a test is not needed, and for writing to your general doctor the same day rather than a fortnight later. He teaches the cardiology rotation for resident physicians and sits on the hospital's chest-pain pathway committee.

What we talked about
Margaret's HbA1c has come down from 7.1% in May to 6.8% now, which is the fourth fall in a row and puts her under the program target of 7.0. Her blood pressure at home is still averaging around 141 over 86, higher than we would like, and that has not moved since the last review. No low-sugar episodes, no dizziness, no swelling in the ankles. The foot check in July was normal.
What she advises
- Carry on exactly as you are with the metformin and the atorvastatin. Nothing about those changes.
- The amlodipine goes up from 5 mg to 10 mg, one tablet each morning. Swollen ankles are the usual side effect — if they appear, ring the department rather than stopping it yourself.
- Keep taking the blood pressure twice a week, in the morning before the tablets, sitting down after five minutes' rest.
- Nothing needs to change about food or walking. What Margaret is already doing is working.
